SCHOOLS, PEERS, AND MEDIA INCES ON CHILDHOOD BEHAVIOR AND BELIEFS

Schools, peers, and the media can impact a child’s development. In schools, teachers play a role in encouraging academic success and student involvement. The characteristics of the school itself also can influence student involvement. For example, larger schools increase the sense of anonymity in students, and teachers are often hard-pressed to provide individualized instruction.

Peers aid in socialization, often through the act of play. Even young children begin to prefer to play with other children rather than play by themselves or with adults. Children often model their behaviors based on their peers. Peers can reinforce behaviors (which may be desirable or undesirable by caregivers or teachers), encourage socially appropriate behavior and attitudes, establish age-related values, and help children learn how to interact with others.

Although television and electronic media impact a child’s development, keep in mind that they only provide the medium for delivery of the message. The medium itself is neither inherently good nor bad. However, the harmful effects of television content (e.g., violence can increase aggressive behavior in viewers; stereotyped portrayals of ethnic and minority groups can leave viewers with inaccurate beliefs about these groups) are more readily recalled than the beneficial influences of some content (e.g., programs such as Sesame Street teach basic cognitive and pro-social skills to their young viewers).

An initial assessment of Ms. Jones’s medical and obstetrical history is as follows.

Obstetric/Gynecologic (OB/GYN) history: Uncomplicated spontaneous vaginal delivery at 39.2 weeks (3 years ago); Cesarean section x 1 at 37.5 weeks for non-reassuring fetal heart tones (1.5 years ago); abnormal Papanicolau (PAP) smear x2, + human papilloma virus (HPV), colposcopy within normal limits

Medical history: Chronic hypertension (HTN) x 5 years;

Allergies: Penicillin

SUBJECTIVE:

Samuel, a 48 – year – old male, presents to the office with mild – to – moderate chest pressure with radiation to his back. Samuel reports that he was awakened from sleep at 7:00 a.m. with chest pressure, initially described as soreness across his anterior chest and through to his back. He rates his pain + 6/10. He felt as though, if he could just belch, he would feel better. His wife drove him to the office to be here when it opened at 9:00 a.m. She tried to convince Samuel to go to the emergency room; but he emphatically refused, insisting on going to the office first. Upon arrival at the office, you take Samuel back to an examination room and instruct the receptionist to call 911. 

Past medical/surgical history: Diabetes mellitus type 2. 

Family history: He has a family history of premature coronary artery disease. His father died of acute myocardial infarction ( AMI ) at age 45. One brother died of AMI at age 49. Social history: He has smoked for 25 years but has reduced his smoking to 1 pack per day since his brother’s death two years ago. He has put on 25 pounds in the past two years and is generally sedentary. 

Medications: Samuel was diagnosed with type 2 diabetes last year. He has been fairly well controlled with diet and Metformin, 500 mg daily. His last hemoglobin A1C was 7.4 two months ago.

General: He is anxious and showing signs of chest pain as you enter the office room. He is slightly diaphoretic. He took an oral aspirin on the way to the office.
