 In response to your peers, provide feedback on a viewpoint that differs from yours. 

1#  I’m in total agreements with not blending friendships with clients. It is important to not mix the professional relationship with friendships as stated in APA codes of ethic for healthcare professionals. Though some counselors will develop special relationships with clients in which the have great compassion for, the overall importance of protecting your personal life from clients, to ensure that the Code Of Ethics are not crossed. When a client engage in a friendship with a counselor, the treatment serious, and not receive the benefit of counseling, and therapist may misuse their power, exploiting clients for self benefit. Therefore, a therapist must maintain proper work decorum, keeping the relationship with clients professional. In addition, the success of treatment greatly depends on creating these boundaries to maximize the effectiveness for therapeutic approaches for treatment. In spite of the fact, blending friendships is not illegal, but seen as unethical, which may create a lot of problems for a counselor both professionally and personally, and affect the outcome for improvement in the client treatment. Becoming friends with clients can also result in disciplinary action by the organization in which you are employed, be seen as an ethical lapse, and a possible disservice for a clients success rate in session. In addition, losing your credentials isn’t worth all the time and effort your put into your studies for the friendship with a client. Although, opening up to clients and given them a sense of comfortability, give client the courage to open up more to counselors, and view their counselor as a confidant, mixing business with pleasure is never good, and at some point cross the line of ethics and boundaries.

What are your boundaries for clients and creating relationships?

2#  Counselors should not be friends with their clients. Once this line is crossed the counselor/client relationship can never be the same. Your client should only be sharing their issues and concerns, and once a counselor becomes a friend they also start sharing their own problems. A client can no longer focus on themselves exclusively. “Psychotherapy is by necessity an imbalanced relationship (Holmes L. Phd 2022)  Being friends with a client creates a dual relationship and therefore does not allow the client to feel that the counselor is safe, and caring and wants to help you figure out your problems not their own.

The Patient

An 18-year-old male was a high school student, lived in a community of approximately 20,000 people, and was a member of the high school basketball team. He died after the car in which he was a passenger struck a tree in a high-speed crash. The driver and two friends were returning from lunch-break in town. The accident occurred just outside school grounds at 12:48 pm. The driver and the back seat passenger, taken by ambulance to the hospital, were later released with minor injuries.  The front seat passenger was pronounced dead at the scene. A reporter from the local newspaper interviewed the schools nurse practitioner, who disclosed the details of the accident, along with some sensitive health information, without authorization by the provider or the deceased’s family. Several weeks later, for educational purposes, this information was included in the Adult Education – Driver Safety Course offered at the town’s Public Library (a cooperative initiative between the high school and adult community). Although in the driver safety class the young man’s name, birthdate, and social security number were not documented anywhere, most people in this modest-sized community were aware that the example case discussed in the course, was in fact not only true, but certain that the case was 100% about this college bound student, who tragically died.

One of your first tasks is to review the federally posted Security Risk Assessment (SRA) on HealthIT.gov.  Then, review the patient case described above with the risk assessment in mind. No, you do NOT have to fill out this risk assessment. Simply familiarize yourself with it. If any specific required data points stand out to you, you may refer to them in your discussion board posting. 

In an initial post, address the following:

   1    If you were the lead advanced practice nurse for the entire regional school district and immediate boss of the high-school advanced practice nurse in question, how would you handle communication and improvement strategies?  

   2    If the school advanced practice nurse clearly removed all of the HIPAA Safe Harbor identifiers (the 18 identifiers in the provision) when answering the news reporter interview questions, is it possible that the deceased’s personal health information would still be exposed?

   3    Considering the educational intention of the cooperative program between the high school and local library, who do you believe is accountable for sharing the private health information of the deceased that is included in the driver’s safety course? Why?

Consequently, you have been asked to select one of the potential improvement projects you noted in your analysis and create a full, 5–10-year strategic plan to achieve the desired quality and safety improvement outcomes. You will develop your strategic plan, using either an AI approach (addressing the design stage), or by building on your SWOT analysis and applying a strategic planning model of your choice.

How you structure your plan should be based on whether you are taking an appreciative inquiry approach or using a specific strategic planning model. Regardless of the approach you choose, the requester of the plan has asked that you address the key points outlined below in the strategic plan requirements.
